Using the new member applications, fill in the new members information into the Membership Submittal Excel Spreadsheet and send member requests for transfer to the new Section to your staff representative in the Member Services Department. Please complete the fields requested in the spreadsheet. The first row in yellow is an example, please do not use. Please do not modify the columns or rows. The highlighted columns in green are required. The ones in white are optional. Enclose all the documentation along with a cover letter identifying the formative Section. (Chose a name for the Section before having the applications completed. Please note: Do not use abbreviations. The staff representative will assign the section a five letter code.)
