  • Format of the form has been followed.
  • Data is presented neatly. Neatness counts and can be invaluable in the ease of evaluation.
  • Citation or reason for the nomination is stated clearly.
  • Specific service award criteria (such as required years of service) has been carefully considered.
  • ISA participation is arranged in chronological order.
  • Other achievements are documented completely.
  • Attachments are referenced to specific portions of the nomination form.

Some Honors & Awards subcommittees have developed judging criteria that is used in their evaluation of candidates. The Committee recommends review of the evaluation criteria prior to submission of a nomination.
