Nomination Forms

  2. Select the award that best fits the accomplishments. Please note: the Honors & Awards Committee may choose to consider the candidate for an award other than the one selected by the nominator based on the Committee’s evaluation of the accomplishment.
  3. Society Officers are not eligible to receive awards during their term of office.
