ISA Fellow Candidate Nomination Materials

The following materials are provided to facilitate your nomination of a Fellow candidate:

1. Nomination Form
2. Letter to Evaluators
3. Evaluation Form
4. Fellows Listed by Expertise and listing of current Admissions Committee Members

The Nominator will identify qualified evaluators and send them the Evaluation Form along with a cover letter and a copy of the nomination form. A helpful action is to provide a stamped, addressed envelope to the evaluator. This will facilitate getting the original copy to the Admissions Committee (headquarters address).

Be sure to keep a copy of the documentation for your files.
